You are not receiving survey access codes for your families this year as they will use their child's OSIS number to access the survey.
***Families will enter a lower case "f" followed by their child's nine-digit Student Identification (OSIS) Number to access the survey (e.g. f123456789). Student OSIS numbers can be found on student report cards or in the family's NYC Schools Account (NYCSA).
